Please see the objectives below for our July 25th program: • Define Palliative Care and identify patients in which Palliative Care is appropriate • Describe the current prevalence of potentially inappropriate medication, or polypharmacy, in patients with serious illness at the end of life • Identify the appropriate population for deprescribing • Explore the current literature base for desprescribing and proposed tools/algorithms • Review the role of motivational interviewing and shared-decision making in the process of de-prescribing
