By attending this program, participants will: 1. Acquire new knowledge and strategies for expanding primary care practice capacity for the continuous improvement of healthcare quality and cost. 2. Explore how Patient Centered Medical Home (PCMH) approaches can benefit practices through enhanced efficiencies, increased reimbursement and referrals, and better staff/ provider satisfaction. 3. Learn strategies to strategically position a primary care practice for success as the healthcare marketplace shifts to value-based reimbursement. 4. Learn new approaches to engage practice leaders and staff in decision-making and shared practice improvement efforts. 5. Actively engage with experts on NCQA PCMH Standards related to improving access to care, team functions, population health, care management, test/referral tracking, and the quality improvement infrastructure necessary for successful primary care practice redesign 6. Celebrate our collective accomplishments!
